New Student to AAPS Enrollment Process at Skyline HS

Welcome to Skyline High School, where the Skyline I.D.E.A. of Integrity, Diversity, Equality, and Academic Innovation fly high.  After completing this enrollment process you will become an official Skyline Eagle!

NOTE:  Please complete the following steps only if you are new to Ann Arbor Public Schools.  Your child has not attended Ann Arbor Public Schools in the previous school year in which you plan to enroll your child for attendance.

In order to enroll your child at Skyline High School you will need to complete the following process. Click the underlined links for more details. You may download a five page summary of enrollment and immunization requirements here. You will need the Adobe Acrobat Reader to open and use this summary.

Step I: Once you have determined that you live within the Skyline boundaries (AAPS Attendance-Area-By-Street Directory- http://curriculumdb.aaps.k12.mi.us:591/streets/) OR you have been accepted for a Skyline High School Open Enrollment seat by the set deadlines, complete the following enrollment packet and deliver or send the information to:

  1. Proof of Residency - you must provide at least 3 articles of adequate proof of where you currently reside. A copy of a sales or lease agreement, deed, utility bill, income tax forms, employment or payroll receipts, insurance documents, physician or medical records, friend of the court or social service records are all acceptable. P.O. Box addresses are NOT acceptable.
  2. Birth certificate/Passport - You must provide a copy of your birth certificate at the time of enrollment. (See the information sheet in the packet for other reliable proof which is accepted.)
  3. Guardianship - You must provide proof of guardianship if you are currently residing with someone other than a parent or relative. If you do not have proof of legal guardianship, you must contact Jane Landefeld at 994-2223, for clearance to enroll.
  4. Immunization -By state law, we must have our certified immunization record in order for you to be granted admission. Please refer to the immunization requirement sheet; if the student needs any immunization(s), please be sure to get them before the appointment.
  5. Records Release Form -This form is sent to your former school to authorize their sending your records to us. Click here for form.
  6. Notice of Entry - Be certain this form is completely filled out (except student number).
  7. Home Language Survey - This form helps us identify students eligible for instruction inEnglish as a Second Language.
  8. High School Credit Check Sheet - Fill out top of page only (except student number). This form is used to evaluate the credits you acquired at your other school.
  9. Health Appraisal - Please complete this form with your doctor and return it to us at the time of your enrollment. Also review the Immunization Data Requirements sheet. You WILL NOT be allowed to attend school if immunizations are not complete. 
  10. Immunization -By state law, we must have your immunization record in order for you to be granted admission.
  11. Transcript/Grade Report(s) from your previous high schools.
  12. Special Education Survey - If your child requires special education services please return this form.
Step II: Complete the forms in the Enrollment Packet.  
  1. Records Release Form - This form is sent to your former school to authorize their sending your records to us.
  2. Notice of Entry - Be certain this form is completely filled out (except student number).
  3. Home Language Survey -This form helps us identify students eligible for instruction inEnglish as a Second Language.
  4. High School Credit Check Sheet -Fill out top of page only (except student number). This form is used to evaluate the credits you acquired at your other school.
  5. Health Appraisal - Please complete this form with your doctor and return it to us at the time of your enrollment. Also review the Immunization Data Requirements sheet. You WILL NOT be allowed to attend school if immunizations are not complete.
  6. Health Information Survey - Please note any physical or personal problems for which the student might require special attention or help from school personnel (e.g. severe allergies, asthma etc.).
  7. Special Education Survey - In order to ensure that your student receives the appropriate evaluations and services, please fill out this form.
  8. Administration of Non-Prescription Medication - Please fill out this form if your student may need to take non-presecription medication at school.
  9. Authorization for School Personnel to Administer Medicine - Please fill out this form if your student may need to take presecription medication at school.
  10. Authorization for Self-Administration of Prescription Medications - Please fill out this form if your student needs to self-administer presecription medication at school.
  11. Allergy Action Plan - If your student has any allergy requiring an epi-pen, please have this form filled out.
Step III: Once all of the enrollment information has been processed, one of the School Guidance Counselors will call you to make an appointment to register your child for courses at Skyline High School.  You will be contacted by a School Guidance Counselor from the SLC office at Skyline High School.  All personnel from the SLC Office can be reached by calling 734-994-7031.  During summer break please call the General Office at 734-994-6515.

The appointment with the School Guidance Counselor will take approximately 45 minutes.

Please be sure to bring the following information to the appointment:
1.  a copy of your previous school courses completed and
2.  a copy of your previous school records of grades earned in the courses,

This information will insure that your child will be placed in the appropriate courses at Skyline High School.  

When making your appointment, please be sure that both the student and a parent or guardian are able to both be present for this appointment.
